Provisional Ballot Affirmation Statements

Provisional Ballots:

Provisional ballots are paper ballots that are used at the polling place on election day under the following circumstances:

(1) If your registration information is missing or is not complete in the poll book.

(2 ) You moved from your registered address to another in the same county and did not register at your new address.

(3) You did not provide the required personal identification information on your registration form or your identification number could not be verified or you do not show it to the poll workers in the polling place (for your provisional ballot, you have until the close of business on the second day after the election to provide your county commissioner of registration with the required ID information).

(4) You requested an absentee ballot, but you never received it.

Affirmation Statement

Prior to voting the provisional ballot or immediately thereafter, the voter shall complete the affirmation statement attached to the provisional ballot envelope, attesting to the information provided in the provisional ballot.
